Adds support for fixed generation times, or generation time distributions without uncertainty - in principle allowing to investigate systematically how such assumptions would affect 1) R estimates and 2) forecasts.

As a bonus it makes the generation time argument optional, so this works:

estimates <- epinow(reported_cases = reported_cases)

Although in that case perhaps no R estimate should be returned.
