Expected behavior

With some long filenames in a directory, columnar output (the default?) can become misleading. Would be preferable to replicate the outpu

Given this XML file:

  <!ENTITY mdash "&#x2014;">

<root>
  &mdash
</root>

There is an error for &mdash because it misses ; we should provide a code action to fix that to insert at the end of &mdash the missing ;

The spec for this can be defined by the implementer
