Description of Change/Update:
Add optional fields to the sponsorship application form requesting State or Country of incorporation.

Reason for Change/Update:
So I can populate the Sponsorship Agreement with the correct state/country of incorporation if it is different from the state/country of the contact address.

Priority: 3. Low

Date Needed: Sponsorship Launch
